“A Childbirth and Childcare Lump-Sum Grant” is paid when an insured person gives birth. “A Dependents' Childbirth and Childcare Lump-Sum Grant” is paid when a dependent family member gives birth. Don't forget to add the newborn as a dependent in these cases.

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Association. For details, check with the medical care institution or other facility where you plan to give birth).
Even if you use this system, you must apply to the Health Insurance Association to receive additional benefits.

The transfer will be made after our confirmation of the use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions. (approximately 3 months after the delivery date).

When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Association in advance.

Applies to: Insured persons and dependents expecting childbirth up to two months in the future who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you paid the entire amount of childbirth expenses to the medical care institution

Submit the following application to the Health Insurance Association if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf or you have given birth overseas.

    (documents demonstrating that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and indicating the name of the Health Insurance Association as application recipient)
  • Copies of receipts for and details of childbirth expenses
    (Documents demonstrating that you have concluded a proxy agreement with the medical care institution or other facility to use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)

Notes: The following documents must be attached for a childbirth overseas:
  • Documents issued by the overseas medical care institution or other facility where the birth took place (birth certificate, receipts, etc).
  • Japanese translations of the above
  • A copy of document(s) attesting to the fact of travel overseas (passport, etc).
  • A letter of consent permitting inquiries with the overseas medical care institution or other facility where the birth took place regarding the fact of the birth, details, and so forth

After the child is born, take the necessary steps to add him or her as a dependent.

Apply for a loan for childbirth expenses

Currently, after an insured person or dependent gives birth, they are paid a childbirth lump-sum allowance, but by applying before giving birth, they then can receive a loan of up to 280,000 yen.

  • Eligible person A: a copy of the Mother and Child Health Handbook or a certificate proving that the expected date of birth is one month or less away
  • Eligible person B: a copy of the mother and child health handbook or a document proving that the pregnancy is at least four months along as well as invoices (receipts) from medical institutions, etc. regarding the costs of giving birth
Submission deadline: As soon as possible
Eligible persons: Insured persons corresponding to (A) or (B)
  • (A) An insured person who is expected to give birth within one month, or a person who has a dependent satisfying that criteria
  • (B) An insured person who is at least four months pregnant and had to make a temporarily payment to a medical institution; or a person who has a dependent satisfying that criteria
Loan amount: up to 280,000 yen
Loan interest: no interest
Loan period and repayment: The period ends on the date when the childbirth lump-sum allowance is to be paid, and the childbirth lump-sum allowance is used to repay the loan (therefore, if a loan is received, the difference of the childbirth lump-sum allowance and loan amount is paid).
Place of submission: Health Insurance Association
Remarks: If you have received a childbirth-expense loan, do not pay directly at the medical institution or other reception desk. (This is because direct payment causes the additional benefit to be less than the loan amount, which results in the need for the insured person to repay the difference).
