Death

"Burial fee" will be paid to those who were financially supported by the deceased insured person, even if only in small part. If there is no such person, a "Burial expense" payment will be made to the person who conducted the burial. In addition, "Dependent's burial fee" will be paid if a dependent dies.

If the insured person has died

Required documents: Burial fee (expense) and Additional benetits application form

In addition to the above application forms, please attach the following documents. Burial fees will be paid if ① to ③ apply, and burial expenses will be paid if ④ applies.

●If the claimant is a dependent・・・①
  • Documents certifying the death
    (Death Certificate and Burial Permit or Cremation Permit (copy) )
●If the claimant is not a dependent but was partially supported by the insured person. In addition to a document certifying the death, the following documents are required.
  • (If living together) Claimant's Certificate of Residence (copy) and the Deleted Certificate of Residence of the Deceased Person (copy) ・・・②
  • (If living apart) documents confirming maintenance of livelihood.・・・③
    for example: copies proving regular remittances (bankbook entries, envelopes from registered cash remittances, etc.), or copies of receipts showing that the deceased insured person paid the claimant’s utility bills.
●If there are no persons who were financially supported by the insured (living separately), the person who actually conducted the burial. ・・・④
  • Documents certifying the death
  • Receipt(s) for burial expenses(original).
